To expedite your initial visit, we ask that you download and fill out our New Patient Packet.

This set of forms will provide us with all the necessary information for the initial consultation.

We recommend arriving at least 30 minutes before your appointment if you have filled out new patient packet before your visit.

We recommend arriving at least 45 minutes before your appointment if you have not filled out the packet beforehand to give you enough time to check in.

We appreciate your assistance and patience in helping us maintain a smooth workflow, and allowing us to provide high quality healthcare in a timely manner.

If you are unable to print or fill out these forms, hard copies will be made available to you when you arrive at your initial appointment.

• Insurance cards
• List of medications currently laking
• Records/referrals if you have access
• Fingerstick Blood glucose readings if diabetic, or CGMS(freestyle or Dexcom) if using
• Old imaging and lab results are always helpful
